Wisdom Tooth Extraction

Wisdom teeth, also known as “molars”, get their name for their tendency to appear completely on the buccal surface when the person is about to complete its development, that is, between 20 and 25 years old. Although there are usually exceptions where they appear in adolescence and even never finish erupting. The two pairs of molars do not always emerge, nor do they appear at the same time. Their early or late eruption can tend to affect the established tooth line, twisting or moving the teeth out of place.


In case the molar has not yet fully erupted, it can create multiple and serious problems in our dental health. Since the location of the wisdom teeth is at the back of the mouth, making hygiene very difficult, which usually favors the proliferation of infections and bacteria in the area; therefore the soft molar tissue usually becomes inflamed and painful cavities appear. In the event that an infection suddenly erupts, the tooth should be removed completely and immediately.


Another reason to consider its extraction is because of the danger it poses to the other teeth around it. In 70% of cases, wisdom teeth are born deformed. In addition to transforming and distorting the correct position of the dentition, a deformed wisdom tooth can develop caries and necrosis of the white tissue of the other teeth around it. Therefore, its immediate extraction when it shows signs of development prevents future problems.

How are wisdom teeth extracted?

Mainly, the specialist must perform a preliminary assessment. This diagnosis will be taken from a panoramic x-ray, and then an evaluation of the mouth for the location of the tooth and determine the difficulty of treatment. Usually, this extraction procedure is performed in a dental clinic in the hands of a surgeon and under local anesthesia. But there are serious cases where it must proceed under general anesthesia and in the nearest hospital. This will be determined by the specialist, and comes along with two other fundamental principles which are:

 

  • The magnitude of intervention: It should be performed under general anesthesia as long as the four wisdom teeth are extracted. Also in the possibility that the mandibular area is weakened and there is a danger of fracturing or damaging the jaw while the treatment is being carried out.

 

  • Systematic condition of the patient: Some patients have more oral bleeding at the time of surgery; therefore the main precaution is to perform the surgery at the hospital level.

 

If the surgery is performed in a clinic, the patient should arrange a consultation with the specialist in charge, in this case, a surgeon, and discuss the conditions of the extraction. The surgeon will explain the administration of local anesthetic with constant punctures in the area, and subsequently, the extraction of the wisdom tooth.

Types of wisdom tooth extractions.

  • Simple wisdom tooth extraction: In the best of cases, the tooth will erupt completely and the extraction will be quick and easy. There are no major problems, and anesthesia is not necessary.

 

  • Extraction with semi-surgery: The wisdom tooth has partially erupted; therefore it is essential to carry out a small, not so laborious surgery to remove the tooth entirety.

 

  • Extraction with surgery: This is the most serious case, and it is when the tooth has not yet fully erupted but tends to bother. In this case, general anesthesia is used and a part of the gum must be opened to remove it completely.
What to do before the extraction of wisdom teeth?

Before the extraction, some details must be taken into account that varies between the medical recommendations and the patient:

 

  • Taking antibiotics: Depending on the patient, the specialist may recommend taking antibiotics days before the operation. This is common in people with a previous illness such as diabetes, low immune system, heart problems, steroid addicts, etc. The use of antibiotics is to prevent germs that reside in our mouth from passing into our blood while the surgery is in process.

 

Taking anticoagulants: Like antibiotics, you have to evaluate their use in the surgical process. Depending on the severity of your treatment, it will be withdrawn or not. Dental procedures are not usually related to blood, but there are exceptions depending on the type of treatment. Each dentist will evaluate it on a personal basis, but it should not be something you should hide from your dentist.

Basic care after the extraction

  • Remove the gauze one hour after the intervention: The purpose of the gauze is to stop bleeding; therefore, if it remains longer on the exposed wound it can create alveolitis. This is a dental infection caused by bacterial culture from the blood that has already coagulated. The gauze can be changed, readjusted, but never kept in the same place for more than 2 hours. If the bleeding does not stop after changing the gauze for 3 or 4 hours, you should go back to the dentist promptly.

 

  • Do not rinse your mouth in the first 24 hours: After the extraction of the wisdom tooth, a hole will remain in the place where it was placed. This hole is called an alveolus, and after this operation, it is covered by blood clots that prevent uncontrolled bleeding and promote tissue healing. Constant rinsing causes its detachment and can lead to hemorrhages and even infections.

 

  • In case of inflammation of the area, do not apply heat: The recommendation for this type of inconvenience is to wrap an ice pack in gauze or tissue and apply it on the cheek for a couple of minutes. If it remains, apply it several times a day. But not more than 4 minutes each application, as it can generate an infection.

 

  • Avoid hot spots: After the operation, it is essential to be placed in cool environments, away from heat. Heat can help to increase inflammation, and therefore generate infections inside the socket.

 

  • Avoid tobacco and alcohol consumption: These two components are incompatible with medicine. Tobacco intake can superimpose the temperature of the mouth; therefore the clot in the socket will melt and cause immediate bleeding. Similarly, tobacco smoke has harmful, highly toxic components, which will generate an infection and even necrosis in the first 48 hours.

 

  • Care when brushing the area: Hygiene after the extraction must be delicate and supervised in detail. As mentioned before, it is best not to force or spit out the remaining water, an alternative for this is to let it drain for at least the first 24 hours after the operation.

 

The bristles should not touch the remaining hole for the first two days, and brushing the tongue should be thorough as the amount of blood coming from the wound can create a bad taste and putrid breath.
